Creating a New Form in WPZOOM Forms

This article refers to WPZOOM Forms

When you install WPZOOM Forms, an Example Form (a simple contact form) is created for you, so you can embed something right away. New forms start from the Choose a template screen: pick one of the ready-made templates, start from a blank canvas, or describe the form and let AI build it (PRO). This short guide covers that first step; the editing itself is covered in Getting to Know the WPZOOM Forms Builder.

Start a new form

In your WordPress dashboard go to WPZOOM Forms → Add New Form (or click Add new form above the forms list). The Choose a template screen opens:

  • The left column lists Blank Form, Generate with AI (PRO) and the templates; use the search box to filter by name.
  • Clicking a template shows a live preview on the right so you can compare before committing.

Option 1: use a template

  1. Click a template — for example Event RSVP — and check the preview.
  2. Click Use This Template in the top-right corner.

The form opens in the builder with all fields in place; edit labels, add or remove fields, and configure notifications as needed.

Templates cover the common cases: Contact Form, Contact Form (Detailed), Newsletter Signup, Quote Request, Feedback, Event RSVP, Appointment Request, File Upload, Job Application, Support Request, Consultation Request, Table Reservation, Customer Satisfaction Survey, Bug Report, Feature Request, Membership Application and more — 24 templates in WPZOOM Forms (free), 35 in PRO. Templates marked PRO in the free plugin need WPZOOM Forms PRO; a few (Event RSVP, Appointment Request, File Upload, Job Application…) use PRO-only fields such as Time or Upload.

Templates are starting points: once a form is created it is independent of the template, so you can change anything.


Option 2: start from a blank form

Click Blank FormStart Blank. The builder opens a minimal starter form called New FormYour Name, Your Email, Your Message and a Send Message button — which you can rename, trim or extend by clicking or dragging fields from the left palette.

Option 3: generate it with AI (PRO)

Click Generate with AI, describe the form in a sentence or two (“a volunteer sign-up form with name, email, phone, preferred days and a message”), press Generate Form, review the preview, then Use This Form or Regenerate. Details in How to Generate a Form with AI.


Name, save and embed

  • Name the form — new forms are called New Form (or the template name). Click the title in the top bar (pencil icon) to rename it; the name is used in the forms list, in Submissions and in notification emails.

  • Save — click Save in the top bar (or Ctrl/⌘ + S). Forms are live as soon as they are saved; there is no separate “publish” step.
  • Embed — click Embed to copy the shortcode ([wpzf_form id="…"]) or use the Contact Form by WPZOOM block / Elementor widget. See Adding Forms to Pages, Posts, and Widgets.

Before you embed, glance at Notifications in the top bar: by default submissions are emailed to the site admin address; switch to Email + Database if you also want entries stored under WPZOOM Forms → Submissions.


Other ways to get a form: duplicate or import

  • Duplicate (PRO) — in WPZOOM Forms → All Forms, hover a form and click Duplicate to create a copy with all fields and settings. Handy for variations of the same form.
  • Import (PRO)WPZOOM Forms → Import & Export accepts a JSON export from another site running WPZOOM Forms. See How to Import/Export Forms.
